On June 12th, initiated by the German Knee Society (DKG), the first International Knee Day-Scientific Paper Online Meeting was held with great attention from all over the world. The three scientific Chairmen: Prof. Dr. Wolf Petersen, Priv.-Doz. Dr. Sven Scheffler and Priv.-Doz. Dr. Thore Zantop invited 35 colleagues from 17 countries to give presentations on the latest research as well as clinical experience on knee.
The meeting was globally live broadcast from 15:00 to 23:00 on June 12th, China time. Four sub-venues was set in China, including Henan Provincial Third People’s Hospital, Zhengzhou Orthopedics Hospital, Guiyang Orthopedics Hospital and Nanchang Hongdu Hospital of Traditional Chinese Medicine.
Henan Provincial Third People’s Hospital
Zhengzhou Orthopaedics Hospital
Guiyang Municipal Orthopedics Hospital
Nanchang Hongdu Hospital of Traditional Chinese Medicine
Six Chinese orthopedic surgeons were invited to give presentations on the meeting: Dr. Yu Sha from Henan Provincial Third People’s Hospital, Dr. Yingzhou Hou, Dr. Wenzhong Chen, Dr. Jinliang Wang from Zhengzhou Orthopedics Hospital, Dr. Huiming Hou from Nanchang Hongdu Hospital of Traditional Chinese Medicine and Dr. Yun Ye from Guiyang Orthopedics Hospital.
Zhengzhou Orthopaedics Hospital
On behalf of Zhengzhou Orthopaedics Hospital, Dr. Yingzhou Hou initially gave a presentation on the topic: “Clinical effect of unicompartmental knee arthroplasty and high tibial osteotomy with tomofix internal fixation in the treatment of unicompartmental knee osteoarthritis”. Unicompartmental knee arthroplasty (UKA) and high tibial osteotomy(HTO) are extensively used in treating Osteoarthritis of unilateral knee. However, the differences of the efficacy between these two approach is still unclear. Fortunately, they found that the both UKA and HTO combined with tomofix internal fixation could improve the knee joint function and symptoms via comparison of the postoperative follow-up of patients treated with UKA and HTO through a randomized controlled study, but postoperative joint mobility of patients who went through HTO is better than that of the UKA, according to Dr. Hou. After the presentation, Dr, Hou and experts both at home and abroad launched in-depth discussion on this topic.
Following, Dr. Wenzhong Chen gave a presentation on the topic:“Influence of Preoperative weight on the early and middle outcome of fixed platform medial unicompartmental knee arthroplasty”. Dr. Chen pointed out that obesity is a high risk factor in osteoarthritis of knee. Moreover, BMI ≥30 kg/m2 will be considered as absolute contraindication. “By analyzing and summarizing the postoperative efficacy of UKA with different BMI, we found that UKA with different BMI can achieve better efficacy, but for obese patients, we need to pay attention to the complications such as poor incision healing.” said Dr. Chen.
Dr. Jinliang Wang gave a presentation on the topic: “Effect of tranexamic acid on the timing of drainage tube removal in total knee arthroplasty”. according to Dr. Wang, placement of drainage tube after operation is crucial. The drainage can effectively reduce the risk of hematoma after knee arthroplasty. However, a long-time placement of drainage tube will increase the risk of postoperative infection. Previously, 24 hours had been regarded as the time mark of drainage tube extraction. We not only effectively reduced the amount of bleeding, but also shortened the time for drainage tube extraction from 24 hours to 18 hours, effectively reducing the risk of postoperative infection. At the end of the report, the Chairman of the meeting gave a detailed comment on the details of the combined application of tranexamic acid.
Besides the wonderful presentations given by the above surgeons, there were also audio sci-graphic being displayed on the conference provided by the following surgeons from Zhengzhou Orthopaedics Hospital:
“Limited Release Medial Collateral Ligamet” -- Jinliang Wang
“Effects of Tranexamic Acid on Ischemia - Reperfusion Injury Caused by Tourniquet in the Surgery of TKA”--Xiaofei Luo
“The Application of Patient Specific Instrument (PSI )in Total Knee Arthroplasty with Valgus Knee”-- Yuehui Wang
Henan Provincial Third People’s Hospital
Dr. Lei Yan, on behalf of the team of Dr. Yu Sha, gave the presentation titled “Clinical effect of open-wedge high tibialosteotomy in treatment of kellgren-lawrence IV knee osteoarthritisassociated with varustibialdeformity”. According to Dr. Yan, open-wedge high tibial osteotomy (OWHTO) has been widely applied for medial knee osteoarthritis with varus tibia. However, there are few and inconsistent reports on the clinical effects of Kellgren-Lawrence IV medial knee osteoarthritis with varus tibia. The study was to compare the HSS knee function scores and complications before and after OWHTO for the patients who suffered from Kellgren-Lawrence IV medial knee osteoarthritis with varus tibia. The team of Dr. Sha found that the patients gave positive feedback with improved knee function, few complications and ability of heavy physical labour. After the presentation, Dr. Yan had further discussion with the international experts on the topic.
As a sub-venue of the International Knee Day, Henan Provincial Third People’s Hospital invited Henan Provincial TV Station to broadcast the meeting and has gained over 82,000 hits online so far!
Nanchang Hongdu Hospital of Traditional Chinese Medicine
Dr. Huiming Hou from Department of Sports Medicine, Nanchang Hongdu Hospital of Traditional Chinese Medicine gave a presentation titled “Arthroscopic reduction and internal fixation of avulsion fracture or anterior cruciate ligament from tibial eminence with sutures by four tibial tunnels”. The surgical technique is a brand new one from the team of Department of Sports Medicine, Nanchang Hongdu Hospital of Traditional Chinese Medicine, and it is mainly applied for avulsion fracture of anterior cruciate ligament from tibial eminence, Meyers-McKeever-Zaricznyj type III, type IV. During the process of fixation, it ensures that the fragments are completely embedded in the bone bed. And it can remain the better fixation of “8” cross sutures while solving the problem it leads to, which is that the posterior part of the fragment may be tilted away from the bone bed. Furthermore, it reduces the incidence of malunion or nonunion, shortens the interval between the surgery and functional exercise with better long-term effect.
Guiyang Municipal Orthopedics Hospital
Yun Ye from Guiyang Municipal Orthopedics Hospital had make a presentation on the topic of A retrospective comparison of the effect of eMP CS and traditional PS prosthesis on ERAS of total knee arthroplasty. Their team perceives that eMP-CS prostheses can help patients to achieve better recovery after TKA.
This study was a retrospective comparison between 2 groups of patients having unilateral TKA(48MP-CS-TKA vs 52 PS-TKA) from Jan 2019 to Mar 2020. The main factors were compared in 2 groups, including Visual Analogue Scale(VAS) pain score,HSS score,FJS-12,range of motion(ROM) and incidence of perioperative complications,as well as overall satisfaction.
The experiment result shows that both eMP-CS prostheses and traditional PS prostheses can help to achieve ERAS after TKA. All patients get a satisfying knee function, and the incidence of perioperative complications was low. Nevertheless,it is more natural for eMP-CS prostheses patients to walk stairs and stand up.
