The conference focused on pelvic and acetabular fractures, a challenging topic in orthopedic trauma. We specially invited Prof. Lehmann, the Director of Trauma Surgery, Orthopedics and Plastic Surgery at the University Medical Center Göttingen and President of the German Society for Trauma Suergery 2027, and brought together numerous orthopedic experts and colleagues from Zhuhai. Together, they engaged in discussions on cutting-edge concepts and technical innovations in the field of pelvic and acetabular fracture, establishing a high-quality platform for academic exchanges to enhance the clinical care capacity of orthopedic trauma in Zhuhai.

The conference was chaired by Prof. JIANG Yuwen, Director of the Separtment of Orthopedic Trauma at Zhuhai People’s Hospital. Invited speakers including Prof. Lehmann, Dr. ZHANG Meiren from the Orthopedics Department at Guangdong Provincial Hospital of Chinese Medicine, Zhuhai Hospital, Dr. HUANG Zhiming from the Department of OrthopedicsⅠat the Fifth Affiliated Hospital of Zunyi Medical University, Zhuhai, Dr. ZENG Langqing from the Department of Orthopedic Trauma at Zhuhai People’s Hospital, and WANG Cheng, delivered insightful case presentations. Focusing on the key issues and challenges in pelvic and acetabular fracture, they shared valuable experiences and academic insights regarding surgical approaches, complication management, and the treatment of associated injuries, drawing on clinical experiences and the latest research findings. The conference featured a vibrant academic atmosphere.

Prof. Lehmann delivered a keynote lecture titled “Combined Pelvic and Acetabular Fractures”. He presented six classic cases of pelvic and acetabular fractures and systematically elaborated on differentiated clinical case strategies as well as individualized surgical protocols tailored to distinct clinical scenarios based on each patient’s specific condition.

Prof. Lehmann concluded that combined fractures represent a significant injury pattern, presenting both the resuscitation challenges associated with pelvic injuries and the complexity of acetabular fracture reduction, making treatment highly challenging. Accurate initial reduction of posterior pelvic ring injuries is critical for treatment, as it directly impacts the effectiveness of subsequent management and the prognosis. Meticulous preoperative planning and a comprehensive assessment of the patient’s condition are essential to reduce the incidence of complications, achieve optimal fracture reduction, and ensure functional recovery. Such combined fractures are often associated with high-energy trauma, and patients may develop serious complications such as hemorrhagic shock, urethral or bladder injuries, and nerve injuries; these potential issues must be addressed concurrently during treatment.

Following the presentation, Prof. JIANG Yuwen, Director of the Department of Orthopedic Trauma of Zhuhai People’s Hospital, and Prof. Lehmann engaged in an in-depth discussion on the challenges in the management of pelvic and acetabular fractures, focusing on the treatment of complex cases involving cranialized displacement of the sacroiliac joint combined with H-type sacral fracture.

Prof. Lehmann shared his insights based on the clinical practices in Germany: Reduction with the spinopelvic instrumentation combined with intraoperative imaging is more convenient and precise than conventional plate-and-screw fixation techniques. Long iliac screws paired with specialized surgical instruments can be adopted for angular adjustment during primary reduction. From a biomechanical perspective, screw fixation alone suffices for isolated sacroiliac joint dislocation; however, stronger fixation is required for concomitant sacral fractures to avoid post-traumatic pseudoarthrosis. In cases of unstable sacral fracture combined with dislocation, favorable reduction with screw fixation notwithstanding, the risk of pseudoarthrosis remains substantial. Clinically, multiple cases of fracture nonunion and recurrent dislocation following sole screw fixation have necessitated revision surgery, which warrants close clinical vigilance.

Dr. ZHANG Meiren from Guangdong Provincial Hospital of Chinese Medicine, Zhuhai Hospital delivered a presentation titled “A Rare Case Report: Robot-Assisted Percutaneous Iliac Screw Fixation for the Treatment of a Late-Onset Sacroiliac Joint Infection Following a Tile C2 Pelvic Fracture,” in which he provided a detailed account of the clinical care process for this rare case and shared valuable clinical experience.

Towards the end of this conference, Prof. Lehmann and Prof. JIANG Yuwen’s team conducted a discussion on a case of pelvic and acetabular fracture that had been admitted to Zhuhai People’s Hospital the day before the conference, following a referral from a hospital in Hongkong. After careful imaging review and comprehensive patient condition assessment, all participants had an in-depth discussion to determine the optimal treatment plan for the case.

After the conference, Prof. JIANG Yuwen, the Conference Chair and Director of the Department of Orthopaedic Trauma at Zhuhai People’s Hospital, remarked in his closing summary that the successful convening of the Sino-German Summit on Pelvic and Acetabular Fractures has delivered high-standard academic exchanges for orthopedic medical staff in Zhuhai, enabling them to access internationally advanced clinical care protocols for pelvic and acetabular fractures. Moreover, it has boosted standardization and technical innovation in the clinical care of pelvic and acetabular fractures in orthopedic trauma between China and Germany.

The conference was held in a hybrid online-offline formats. All attendees commented that they had learned a lot from it and looked forward to more such academic activities in the future to jointly advance the development of regional orthopedic medical services.
