On June 19th, 2020, the University Hospital of Munich held a Sino-German Joint Surgery online video conference together with Dongguan Hospital of Traditional Chinese Medicine, Dongguan People’s Hospital, Dongguan TungWah Hospital and Donguan Eighth People’s Hospital.
This conference was led by Guohua Ye, Medical Director of Dongguan Hospital of TCM, and Prof. Dr. med. Volkmar Jansson, Director of the Clinic and Polyclinic for Orthopedics at the University in Munich, Klinikum Großhadern. The host was Zhongqiang Huang, Head of General Surgery and Joint Department of Dongguan Hospital of TCM. The attendees include Zhichao Tan, the Associate Head of General Orthopedics Department of Dongguan Hospital of TCM, Xueming Zhou, Head of Orthopedics of Dongguan People’s Hospital, Guanghui Chen, Head of Joint Surgery Department of Dongguan TungWah Hospital and Jiande Meng, Head of Bone and Joint Department of Dongguan Eighth People’s Hospital. Consisting of 2 lectures and 5 cases discussion, the conference was held in an elevating atmosphere.
Prof. Dr. med. Dipl.-Ing. Volkmar Jansson, the Director of the Clinic and Polyclinic for Orthopedics, Physical Medicine and Rehabilitation at the University in Munich, Klinikum Großhadern, is the specialist on hip and knee primary replacement and revision as well as on repairing ligament and cartilage.
In his speech, Guohua Ye, Medical Director of Dongguan Hospital of TCM stated that it was quite an honor to have invited Prof. Jansson, Director of the Clinic and Polyclinic for Orthopedics at the University in Munich, Klinikum Großhadern, to conduct an online course. It would surely be a precious feast of academic exchange with, bringing the Orthopedic development of Dongguan city to a new level.

On the conference, Prof. Jansson share with us of two topics. Firstly, he introduced the EPRD - the German registry for hip and knee replacement, saying that this registry is the Biggest data set in Europe for this kind of surgical procedures. There are 1.25 million prosthesis being registered in it, 80% of German hospitals upload prosthetic replacement data in this database. Revision rates of 400,000 primary implantations with follow up of 4 years are also being taken down in it, so this database is valuable for the accessing of the primary replacement and revision surgeries on hip and knee.
Prof. Jansson sharing his the topics
Prof. Jansson shared with us a topic on The key points on treating the postoperative infection of primary hip replacement. He then instructed us with how to prevent surgical site infection, how to differentiate the stage of infection, and the diagnosis and treatment points on PJI infection. Prof. Jansson also shared with us the DAIR (debridement, antibiotics, implant retention) procedures. The success rate of DAIR procedures on treating infection can go from 18% to 100%. While Prof. Jansson pointed out that Arthroscopic DAIR and Repeated DAIR are not recommended. If DAIR treatment fails, referring to two-staged revision with removal of implants always is the golden rule.
Prof. Jansson also made it clear on the duration of antibiotic treatment in their hospital. According to guidelines in their hospital, 12 weeks of antibiotic treatment is recommended.
The two lectures of Prof. Jansson’s are all instructive so the fellow surgeons all learned a lot from them.
Zhongqiang Huang, Head of General Surgery and Joint Department of Dongguan Hospital of TCM
Later on, Zhongqiang Huang, Head of General Surgery and Joint Department of Dongguan Hospital of TCM shared a case of Traumatic arthritis on the right hip(advanced stage). all the fellow surgeons had an active discussion on how to orderly loosen, how to reconstruct the center of rotation and how to prepare the prosthesis in this session.
Xueming Zhou from Dongguan People’s Hospital
Subsequently, Xueming Zhou from Dongguan People’s Hospital delivered a case sharing of hip revision surgery. As in these case, infection is hard to identify so Prof. Jansson gave him instruction on doing bio-section and arthroscopy to determine the infection together with six-weeks antibiotics treatment.
Director Guanghui Chen from Dongguan TungWah Hospital
Moving on to the Case Sharing of Director Guanghui Chen from Dongguan TungWah Hospital. He delivered a case of UKA replacement to UKA revision. The patient was diagnosed as infected after UKA, Director Chen removed the prosthesis, performed debridement, put in the spacer and the performed UKA revision weeks later. Prof. Jansson praised Dr. Chen for his treatment.
Director Jiande Meng from Dongguan Eighth People’s Hospital
Then Director Jiande Meng from Dongguan Eighth People’s Hospital shared a case with Charcot joint. Director Meng also discussed with Prof. Jansson and fellow audiences on the timing of surgery, surgical approaches and choice of prosthesis. Prof. Jansson was very positive about Director Meng's choice of technique and implants.
Associate Director Zhichao Tan from Dongguan Hospital of TCM
Finally, Associate Director Zhichao Tan from Dongguan Hospital of TCM delivered a case sharing on hip revision. The patient firstly had swell on her right hip as well as cystic mass on her thigh. And she was diagnosed as prosthetic loosening after primary hip replacement with complication of Type 2 Diabetes Mellitus an so on. After excluding infection on this case, Director Tan gave surgery on this patient and A twenty-month post-operative follow up shows that the patient had recovered well. Prof. Jansson said it was a great case and the treatment procedures and choice of prosthesis and the outcome of the surgery are of success.
At the end of the conference, Director Huang expressed his gratitude towards Prof. Jansson for his excellent courses and patient guidance, which are quite beneficial to those who were present. He thanked Director Zhou from Dongguan People’s Hospital and Director Chen from Dongguan TungWah Hospital for their cases sharing. He also thanked companies of brotherhood for their support and encouragement to the development of the Orthopedics Department of Dongguan Hospital of TCM. Last but not least, he thanked Guangzhou Gloryren Medical Technology Co. for providing this platform for all parties to learn from each other.
